Please complete the constructive unfair dismissal form below, outlining why you felt you had to resign and how your employer was in breach of your employment contract.  For a constructive dismissal claim to succeed, it has to be shown that your employer was in breach of a term of your employment contract, the breach was serious so justifying your resignation and you resigned directly because of that breach of contract.
 
Before you can make a claim for constructive dismissal you should raise a grievance with your employer.  Failure to do this may result in compensation being reduced.
